Acne scars are not one condition

Atrophic scars sit below the surrounding surface and may be narrow ice-pick scars, sharply edged boxcar scars or rolling scars caused by deeper tethering. Raised hypertrophic or keloid scars behave differently. Persistent red or brown post-acne marks are colour changes, not true textural scars.

A person can have several scar types together. Treatment selected from a photograph or package name alone may miss tethering, depth, active acne and pigment risk.

What the assessment considers

The clinician examines scars under directional light and reviews active breakouts, medicines, previous isotretinoin or procedures, skin tone, melasma, cold sores, healing history and tendency to dark marks or raised scars. Standardised photographs help compare change.

Active acne is usually controlled first because new lesions can create new scars. Expectations, acceptable downtime, budget and willingness to complete aftercare influence the treatment sequence.

How treatment options are matched

Microneedling may suit selected atrophic scars with comparatively limited surface downtime. MNRF adds controlled radiofrequency delivery at depth. Fractional CO₂ provides ablative resurfacing with more visible recovery and pigment considerations. Focal chemical reconstruction, subcision, fillers or surgical methods may be discussed where available and appropriate.

Combination treatment is used because one method rarely corrects every mechanism. More procedures do not automatically mean better results; sequencing and healing time are important.

Course, measurement and maintenance

Scar remodelling occurs over months. The number of sessions depends on scar mix, treatment intensity, recovery and observed benefit. Consistent lighting and validated scar grading are more reliable than heavily edited photographs.

Improvement is usually partial. Pores, natural texture and deep scars may remain. Preventing new acne and protecting healing skin from sunlight are central to maintaining progress.

Safety and when to postpone

Active infection, inflamed acne, eczema flare, recent tanning, open wounds or uncontrolled medical conditions may require postponement. Risks vary by procedure and can include pain, bruising, infection, prolonged redness, pigment change, acne or herpes flare, track marks and scarring.
