Scar assessment comes before the device

Acne scars may be ice-pick, boxcar, rolling, tethered, raised or mixed. Fractional CO₂ is not equally useful for every pattern. Deep narrow scars, tethering, active acne and raised scars may require other or combined approaches.

Assessment also covers skin tone, recent tanning, melasma or post-inflammatory pigmentation, medicines, infection, cold-sore history, previous procedures, healing and keloid tendency. The objective is to choose the least risky treatment capable of addressing the dominant scar type.

How fractional CO₂ resurfacing works

The laser treats a fraction of the surface in a grid of microscopic columns while leaving intervening tissue untreated. Controlled ablation and heat initiate healing and collagen remodelling. Density, energy, passes and treatment depth are adjusted rather than applied as one standard setting.

Improvement is gradual and usually partial. The procedure cannot erase scars or create perfectly smooth skin, and repeated aggressive sessions can increase pigment and texture complications.

Procedure and recovery

Preparation may include pausing irritating products and following sun-protection or antiviral instructions when prescribed. Numbing is commonly used. After treatment, warmth, swelling, redness, pinpoint oozing, crusting and peeling may occur.

Visible recovery often takes several days, while redness and sensitivity can persist longer. Gentle cleansing, prescribed barrier care and strict sun protection are important. Scrubbing, picking, swimming and active skin products should resume only when advised.

Treatment course and outcomes

The number and spacing of sessions depend on scar type, intensity, healing and response. Collagen change continues after surface recovery, so judging a session too early can lead to unnecessary retreatment. Standardised photographs and scar grading help set a review point.

Evidence supports fractional CO₂ as an option for atrophic acne scars, but studies vary and comparative evidence does not establish one universally superior procedure. Combining treatments may be considered when different scar mechanisms are present.

Pigment and safety considerations

Post-inflammatory hyperpigmentation is particularly relevant for darker skin tones. Other risks include prolonged redness, infection, acne or cold-sore flare, milia, demarcation lines, lightening, scarring and an unsatisfactory response. Conservative settings and appropriate preparation do not remove risk but may reduce it.

How much downtime should I expect?

Visible redness, swelling, crusting and peeling commonly require several days, while sensitivity or redness may last longer depending on intensity.

Will one session remove my scars?

No responsible plan guarantees removal. Improvement is gradual and often needs staged or combined treatment.

Can darker skin tones be treated?

They may be treated in selected cases, but post-inflammatory pigmentation risk requires careful assessment, conservative parameters and aftercare.
